New Sod Issues

Hello -

I recently replaced the top 4 inches of my hard clay soil and replaced it with top soil and added sod (tall fescue). I have been closely monitoring the sod, it was doing very well and looks green from afar, but up close I see a lot of yellowing spots and areas with matted dead grass. I’m not really sure what the problem is and what to do. It just doesn’t seem like healthy.

Before replacing the soil, my biggest issues was drainage. We regraded the yard when we replaced the soil. I bought a moisture checker and the sod layer has been moist/dry most of the time, but about 3-4 inches deep it always reads wet.

From reading around, I think it’s fungus from too much moisture or thatch. Please help! See link for Pics.
